Insurance: Dr. Baum is not affiliated with any insurance plans. Fees for services are paid by the patient on the day of their visit. A Superbill is given to each patient to enable them to file with their insurance company for reimbursement. Most of Dr. Baum’s services for Orthopedic Medical Procedures have codes acceptable by your Insurance Company for reimbursement with the exception of HMO’s, PPO’s, Medicare, Medicaid and VA Insurance.  For all insurance reimbursement Dr. Baum is considered an out of network provider.  Dr. Baum has opted-out of the Medicare program as they only cover drugs and surgery. Chelation Therapy is a alternative procedure that will not be covered by any insurance carrier.

Medical Records: All your medical records are considered confidential information. Dr. Baum will release your records but only with your express written authorization.
